Mechanism of action: Erectile dysfunction: Tadalafil and sildenafil belong to the same selective type 5 phosphodiesterase (PDE5), but their structure is different from that of the latter, and a high-fat diet does not interfere with their absorption. Under sexual stimulation, nitric oxide synthase (NOS) in penile nerve endings and vascular endothelial cells catalyzes substrate L-arginine to synthesize nitric oxide (NO). NO activates guanylate cyclase, which converts guanosine triphosphate into cyclic guanosine phosphate (cGMP), thereby activating cyclic guanosine phosphate dependent protein kinase, resulting in a decrease in the concentration of calcium ions in smooth Chemicalbook muscle cells, resulting in relaxation of the smooth muscle of the corpus cavernosum and erection. Phosphodiesterase type 5 (PDE5) degrades cGMP into an inactive product, causing the penis to enter a weak state. By inhibiting PDE5 degradation, Tadanafil leads to cGMP accumulation, which relaxes the smooth muscle of the cavernous body of the penis and leads to penile erection. Since nitrates are NO donors, the combination with tadanafil can significantly increase the level of cGMP and lead to severe hypotension, therefore, clinical contraindication of the two.
