Description

Digoxin is a purified cardiac glycoside similar to digitoxin extracted from the foxglove plant, Digitalis lanata. Digoxin is occasionally used in the treatment of various heart conditions, namely atrial fibrillation, atrial flutter and sometimes heart failure that cannot be controlled by other medication.
There are concerns that its general use may increase the risk of death.
Digoxin was discovered by physician William Withering. It is on the World Health Organization's List of Essential Medicines, a list of the most important medications needed in a basic health system.

General advice

4.2 Most important symptoms/effects, acute and delayed

Material is a digitalis glycoside. Ingestion can cause death. Material is considered super toxic; probable human oral lethal dose is less than 5 mg/kg, a taste (less than 7 drops) for a 70 kg (150 lb.) person. Persons at risk include those taking drugs for thyroid and renal diseases. Quinidine and diuretics taken concurrently with digoxin can be hazardous. It should be used with extreme care during pregnancy and in nursing mothers. (EPA, 1998)

4.3 Indication of immediate medical attention and special treatment needed, if necessary

Treat hyperkalemia if greater than 5.5 mEq/L with sodium bicarbonate, glucose with insulin, or sodium polystyrene sulfonate. Do not use calcium; it may worsen ventricular arrhythmias. Treat bradycardia or heart block with atropine; a temporary pacemaker may be needed for persistent symptomatic bradycardia. Ventricular tachyarrhythmias may respond to lidocaine or phenytoin, or to correction of low potassium or magnesium. /Cardiac glycosides/
